Parent Application

Educational Talent Search (ETS) is a TRIO program funded by the U.S. Department of Education.  We currently provide your school with a variety of workshops designed for 6th-12th grade students.  The following form provides information that satisfies the Department of Education's criteria for participation in ETS.  This data will be kept confidential and is required for continued funding of the program.

Your signature below will certify as to the truth of the statements made on this form and will also serve as an authorization for the Educational Talent Search program to request and receive information from your school (including, but not limited to, period report cards, free or reduced lunch status, social security number, standardization test results, use of photographs including but not limited to the ETS webpage and ETS Facebook and bebo pages, and transcriptions that might be pertinent to the program). It will also imply your permission for the student to take part in all activities related to Educational Talent Search.
